Please provide us with your complete insurance information, and bring your insurance card to all of your appointments. As a courtesy, we will submit the claim on your behalf. If we do not participate with your insurance company, you will receive a monthly statement. Insurance Companies we participate with.
To accommodate the diversity of approaches to health insurance in today's managed care environment, we participate with a large number of managed care plans. The key to the smooth processing of your managed care claim is the Referral Form completed by your Primary Care Physician. To see us for an office visit, you must provide a Referral Form or be willing to sign a Statement of Personal Responsibility. If your plan requires a co pay, this payment is expected at the time of service or a $10 fee will be charged.
If you are injured through your employment or are involved in an accident, we will file your Worker's Compensation or No Fault Insurance claim. You must bring the required information (our staff can assist you) to your appointment so that we may properly process this type of claim. You and your employer's cooperation will help insure timely submission of this claim.
As a participating provider, we welcome Medicare patients and are pleased to submit your Medicare and any secondary insurance claim forms for you.
Our policy requires payment in full at the time of treatment if you are not covered by an insurance plan with which we participate. Regardless of your insurance plan, please understand that you are ultimately responsible for payment.
There is a wide variety of coverage for DME, some plans cover in full, some pay 80% with a 20% co pay, others plans only cover with the appropriate rider. Some plans cover certain items of DME but not all. With all these variables we cannot be expected to know the coverage you have in your insurance contract. It is NYS law that DME cannot be returned.
